How To Register

Follow these steps to register:

1. Download the RCR Registration Form and Registry Participation Agreement.

   Complete one copy of the registration form for the practice and one Clinician Agreement for each provider participating in the RCR and forward them to Outcome by fax (866-728-9151).

3. Download the e-mail RCR Registration Form and e-mail Registry Participation Agreement.

   Complete one copy of the registration form for the practice and one Clinician Agreement for each provider participating in the RCR. Complete in full, save and e-mail to .

2. Send original signed copies of agreements to Outcome by U.S. mail at the following address:

   Outcome Sciences, Inc.
   ATTN: RCR
   201 Broadway, 5th Floor
   Cambridge, MA 02139

3. Within two business days of receiving your fax or e-mail, Outcome will contact you to set up the following:

   • RCR user training for the practice via webinar

   • RCR usernames and passwords

4. Once the practice has received its access to the RCR, users can begin to enter data.

When To Register

The ACR recommends that practices register approximately one month before they intend to start data collection so they can sign relevant agreements and receive training and materials well in advance of data collection.
