The who, what, when, where, why, and how for meaningful use objectives measuring the collection of structured data in the EHR. Measures discussed on this call include:

  • Maintain an up-to-date problem list of current and active diagnoses
  • Maintain active medication list
  • Maintain active medication allergy list
  • Record and chart changes in vital signs including height, weight, and blood pressure
  • Record smoking status for patients 13 years old or older
  • Record demographics including preferred language, gender, race, ethnicity, and date of birth
