Revision of the CMS-1500

The National Uniform Claim Committee revised the CMS-1500 to accommodate the mandated National Provider Identifiers (NPIs). The new version, CMS-1500 (08-05), will replace the current CMS-1500 (12-90) version effective April 1, 2007. CMS will discontinue acceptance of the CMS-1500 12-90 version form on April 1, 2007.

The previous CMS-1500 (12-90) form did not have the fields for reporting of NPIs. Further information on the CMS-1500 form is available through the NUCC web site at http://www.nucc.org/. Download the new CMS 1500 form.
